COMMERICIAL DRIVER'S LICENSE (CDL)
Drug Testing, Physicals and Department of Transportation (DOT) Verification

Under Arizona Board of Regents (ABOR) policy 6-709 and University Policy 103.1, all permanent, full- or part-time, temporary and student employees required to maintain a Commercial Driver License (CDL) to perform their job duties must complete a CDL Drug Screening, Physical, and Department of Transportation (DOT) employment verification.

Note: Only a limited number of University positions (such as law enforcement and shuttle/van driver) require a drug test. Unless authorized by Risk Management and Safety or Human Resources, hiring authorities may not conduct drug tests on new or current employees.

  1. When completing the on-line Career Track job requisition, determine if the position requires a CDL DOT Drug Testing, Physical, and DOT Employment Verification as defined by Risk Management and Safety Policy Number RM-002. Indicate the requirements in the Supplemental Pre-Employment Screening section of the job requisition.


  2. In addition to completing a consent and disclosure form for undergoing a criminal background and Department of Motor Vehicle check, all new CDL drivers should be advised by the department to sign a separate University of Arizona CDL Release Form for each position held as a CDL Driver in the prior three years. Advise the finalist to return the signed copies to the hiring department. This verification provides drug and alcohol testing history from previous employers for the prior three years.


  3. The Department of Transportation (DOT) work history verification must also be completed on all CDL drivers for all positions held in the prior ten years. The candidate consent and disclosure form will serve as consent for conducting the required work history verification should the hiring department choose to use the UA vendor to conduct the ten-year work history check.


  4. The hiring department shall also provide the finalist with an “Authorization for Non-Injury Medical Exam” form, a “Chain of Custody” form and directions to the nearest clinic/testing facility. (See: http://risk.arizona.edu/fleetsafety/index.shtml).


  5. The finalist will be required to contact the clinic/testing facility directly to schedule an appointment.


  6. Risk Management and Safety will review the results of Physical and Drug tests and DOT information and contact the hiring department with a finding.


    1. Non-Adverse Information Discovered: If information received is non-adverse, Risk Management will notify the department that no adverse information was uncovered. Pending the successful outcome of any criminal background checks, new hire documents may be processed through Human Resources as usual.


    2. Adverse Information Discovered: If adverse information is received and confirmed by Risk Management, the Risk Management Coordinator and/or Human Resources will contact the hiring department authority and advise as to appropriate action.
