1. If I'm unable to work because of an illness or injury, how do I file a disability claim?

The UA has several types of disability insurance. All retirement-eligible employees are covered by long-term disability insurance. Benefits are payable after a six-month waiting period for a disability or illness. The disability or illness does not have to be an "on the job" illness or injury for you to qualify for long-term disability benefits. If it becomes necessary for you to end your University employment for medical reasons, you may qualify for disability benefits as long as the disability occurred prior to the date of your employment termination.

In addition to long-term disability insurance, you may choose to purchase short-term disability insurance from two plans offered through the University. The premiums are paid through payroll deduction and are not subsidized by the University.

If you should sustain an injury or suffer an illness that may result in a lengthy absence from work, please contact the Benefits Office at (520) 621-3662 so that we may begin processing your claim. Processing your claim may require that we discuss continuation of other benefits.





2. What happens in the event of the death of an employee?

All benefits-eligible employees are covered by a $15,000 life insurance policy with an accidental death and dismemberment clause.

Human Resources assists surviving benenficiaries to obtain their benefits. This process includes obtaining information required to process the claim and consultation on any other affected benefits. The claim forms and death certificate are then forwarded to the insurance companies.





3. What if I disagree with a formal decision made by my medical or dental carrier regarding coverage or care?

  • Contact the carrier's member services (contacts) to file a formal appeal or grievance.
  • Contact Human Resources to notify them for tracking purposes.
  • Contact the Department of Insurance and the Arizona Department of Administration to register your complaint.
