Welcome to the 2007 Benefits Open Enrollment website.

 

NEW COVERAGE OPTION: EFFECTIVE OCTOBER 1, 2007 FOR THE 2007-2008 PLAN YEAR

In addition to the existing two tiers of coverage ("Employee Only" and "Employee plus Family") a third tier is now available for "Employee plus One Dependent". Employee contribution rates remain unchanged for the existing tiers.

Proof of dependent eligibility will be required unless documentation has been received by UA Human Resources.

All eligible employees must re-enroll for all insurance coverage, even if no change to plan or coverage level is anticipated. Otherwise, medical, dental, vision, voluntary life and short-term disability insurance coverage will end on September 30, 2007.

ADDITIONAL PLAN CHANGES

Annual Mammograms Coverage Expanded
Annual mammogram screenings are now a covered benefit for women beginning at age 40.

Substance Abuse Residential Treatment Expanded
Coverage is now available for voluntary and court-ordered substance abuse residential treatment. Previously, only hospitalizations for those with chemical and alcohol dependency were covered.

Generic and Brand Name Medications
If a physician prescribes a brand name drug, the member will be responsible for paying the appropriate co-pay. If the physician prescribes a brand name or a generic drug, members who select the brand name drug will pay the difference in the actual cost of the two drugs.

Physical Examination Limit Coverage Expanded
Coverage for an annual physical examination has been increased from $250 to $1500, which is intended to encourage thorough examinations for early detection of illness or serious health conditions. There is no change to the fee schedule for physicians and laboratories.

Smoking/Tobacco Cessation Aids Now Covered
To assist members' efforts to quit smoking, the plan now reimburses members for smoking/tobacco cessation aids up to $500 over a member's lifetime.

Co-payments for Emergency Room
Emergency room co-payments are increasing from $75 to $125. Members are encouraged to consider using their network's urgent care center. The co-pay for an urgent care center visit remains at $20.

Physician Specialist Co-Pays
Co-payments for a visit to a physician specialist are increasing from $10 to $20. On occasion, members will seek care from a specialist when a primary care physician would suffice. Members are encouraged to carefully consider the best physician option before scheduling appointments. Primary care physician categories include: general medicine, internal medicine, family medicine and obstetrics/gynecology.
