Understanding, Using and Maximizing Your Benefits
Medical Insurance:
The first point of contact for questions/concerns regarding coverage, claims, policy, identification cards, physician selection or changes is your medical plan administrator. For medical Networks: Arizona Foundation, RAN+AMN (HMA), Schaller Anderson Healthcare, and Beech Street, contact AzBO Harrington at 1-888-999-1459 or, United Healthcare at 1-800-896-1067.
If you are unsuccessful in obtaining the appropriate or necessary information, your next step should be to contact Arizona Benefit Options (AzBO) call center at 1-800-304-3687, located at the Arizona Department of Administration.
Prescription Benefit:
Walgreens Health Initiative (WHI) has a 3-tier prescription benefits program - $10 per fill for generic drugs on the drug list, $20 for brand name drugs on the drug list, $40 for drugs not on their drug list (some medications do require preauthorization from Walgreens Health Initiative (WHI), contact Member Services to determine if a specific drug is in this category).
There are least two ways to save money on prescription costs. The first is to use WHI's Tel-Drug mail-order program. This program allows you to order up to a 3-month supply of maintenance medication for the cost of 2 co-pays. Contact WHI's member services for enrollment or for more information call 1-866-722-2141.
Another good way to save on prescription costs is to use our flexible-spending account. You can designate the money you spend on your prescription costs to be deducted from your paycheck on a pre-tax basis and be reimbursed by that money each time you incur an eligible expense. This will help to lower your taxable income so you will take home more money over the course of the year.
Dental and Vision Insurance:
The first point of contact for questions or concerns regarding coverage, claims, policy, identification cards, doctor selection or changes is your dental carrier's member services.
If you are unsuccessful in obtaining the appropriate or necessary information your next step would be to contact Human Resources for further assistance and/or instructions.
