What is this plan/coverage?
The University of Arizona's Group Long Term Care Insurance plan features five
levels of coverage ($100, $150, $200, $250, $300) and three defined durations
(3-year, 5-year, and Unlimited). Rates are available on the
Prudential website.
Use the Group Name: university
Access Code: arizona
(both case-sensitive), and select the applicant's state of residence.
Plan Highlights:
- No underwriting required for actively-at-work, benefits-eligible employees under age 85 during March 3-26, 2008 Open Enrollment and during first 31 days after new eligibility/new hire (beginning March 3, 2008).
- Simplified underwriting for spouse/domestic partners (same or opposite gender) up to age 65 during March 3-26, 2008 Open Enrollment and during first 31 days of an employee's new eligibility/new hire (beginning March 3, 2008).
- Cash Alternative Benefit provides maximum flexibility for using the cash value of the daily benefit amount
- Premium Refund Available upon Death returns a portion of premiums paid into the plan if the insured dies, based on insured's age at death and prior receipt of benefits
- The Unlimited Lifetime Plans require full underwriting of all applicants. If an actively-at-work, benefits-eligible employee does not satisfy underwriting requirements, coverage will default to the 5-year, $300/day coverage option (Plan 10). Important Note: See Underwriting Requirements
Who is eligible to enroll?
Benefits-eligible employees who are actively-at-work and are employed at a .50 FTE (20 hours per week) or greater and in a position that is at least six (6) months or more in duration, and their
- spouse/domestic partner (same or opposite gender),
- siblings over age 18,
- parents and parents-in-law,
- grandparents and grandparents-in-law,
- adult child(ren) over age 18,
- spouse of adult child(ren) over age 18, and
- spouse or domestic partner (same or opposite gender) of domestic partner's adult child(ren) over age 18.
University of Arizona retirees and their
- spouse/domestic partner (same or opposite gender), and
- surviving spouse/domestic partner (same or opposite gender).
What are the underwriting requirements for this benefit?
Underwriting requirements are determined by relationship, when the application is received by Prudential, age, and level of coverage.
During Open Enrollment, March 3-26, 2008:
For actively-at-work, benefits-eligible employees under age 85 who are hired or become benefits-eligible prior to March 3, 2008.
Actively-at-work, benefits-eligible employees under age 85 may obtain 3-year or 5-year benefit coverage options without any medical underwriting.
An actively-at-work, benefits-eligible employee's spouse/domestic partner (same or opposite gender) under age 65 is eligible for coverage by completing a simplified underwriting application.
During the first 31 days following benefits eligibility
For actively-at-work, benefits-eligible employees who are hired or become benefits-eligible on or after March 3, 2008.
Actively-at-work, benefits-eligible employees under age 85 may obtain 3-year or 5-year benefit coverage options without any medical underwriting.
The Unlimited Lifetime benefit coverage option requires full medical underwriting for all applicants.
An actively-at-work, benefits-eligible employee's spouse/domestic partner (same or opposite gender) under age 65 is eligible for coverage by completing a simplified underwriting application.

When is insurance coverage effective?
The first of the month following date of application acceptance.
Is there an annual open enrollment period?
This plan does not offer an annual guaranteed acceptance open enrollment.

How are premiums paid?
You may chose from convenient electronic funds transfer from your financial institution, or direct billing. Payroll deduction is unavailable.
When may I obtain coverage without any medical underwriting if I am on a leave of absence during the March 3-26, 2008 Open Enrollment period?
Upon returning to work from a leave of absence, a benefits-eligible employee will have 31 days to obtain this coverage without any medical underwriting.
How often are premiums paid?
Premiums may be paid monthly, semi-annually, or annually. Discounts are available for semi-annual and annual payments. Payroll deduction is unavailable.
May I take this insurance coverage with me if I leave The University of Arizona?
You may keep this insurance after you leave University employment, as long as you continue to pay premiums.
Does this product have an inflation protection feature?
The Periodic Inflation Protection feature is included in the base plan. As an optional enhancement, an applicant may purchase the Automatic Inflation Protection feature.

How do I enroll?
To take advantage of special underwriting during Open Enrollment March 3-26, 2008,
actively-at-work, benefits-eligible employees and their spouse/domestic partner
under age 65 (same or opposite gender) are encouraged to enroll online
using the Group Name: university, Access Code: arizona (both case-sensitive), and
select the applicant's state of residence.
The Unlimited Lifetime option requires completion of a paper application.
Retirees and all other family members must complete a paper application.
