Enrollment for University of Arizona Retirees

Retirees must complete a paper application. Application forms may be ordered by calling Prudential toll free at (800) 732-0416 and requesting a University of Arizona enrollment kit. The application forms are also available online using the Prudential Group Long Term Care Insurance website. The Group Name is university and the Access Code is arizona. (Both are case-sensitive.)

Note:  You must use a Windows-based browser, such as Internet Explorer, to complete the Prudential online application.
