State that the employee is being placed on disciplinary probation, and state both the length of time (NOT LESS THAN ONE MONTH NOR MORE THAN SIX MONTHS) and the beginning and end dates of the disciplinary probation period. Give the reason for the discipline and quote the applicable rule violated from Classified Staff Policy #401.0, Rules of Conduct.
For example:
"You are being placed on disciplinary probation for [1-6 months] because of your continued failure to maintain regular and satisfactory attendance and failure to follow call-in procedures. This is in violation of Classified Staff Policy #401.0, Rules of Conduct. Your disciplinary probation will begin on [date] and end on [date] ."
State the facts, give examples, list witnesses, dates, etc. that verify the substandard performance/behavior. Explain the impact that this has had on the operation.
For example:
"During the past month, you have missed three of the six shifts for which you were scheduled, and on each occasion, you failed to notify your supervisor in advance that you would be absent which violates our call-in procedures. These unexcused absences occurred on [dates] . As a result of your unexcused absences, the department has missed 10 shipment deadlines and had to reassign work to others."
Describe any previous problems and corrective actions that are relevant to the current situation.
For example:
"On [date] you were given a verbal warning for unexcused absences and on [date] you were given a written warning for unexcused absences and failing to follow call-in procedures. I have also discussed your unsatisfactory attendance with you on your most recent performance evaluation of [date] ."
Give the behavior and/or performance that must be achieved to be considered satisfactory. State outcomes to the extent possible with specific actions detailed only as necessary. Also, state that once the improved level of performance/behavior is achieved, it must be maintained.
For example:
"This is your opportunity to correct your unsatisfactory attendance. I need someone to successfully fulfill the duties of this position. In order for your performance to be considered satisfactory, you must maintain regular attendance as described in our attendance procedures, have no unexcused absences and utilize the prescribed call-in procedures if you are to be late or absent."
