Medical Leave (Non-FML) - Appointed Personnel
Definition
- The purpose of this leave is to allow female employees up to six months off during their pregnancy and recuperation that would not be covered under the Family and Medical Leave Act.
Eligibility
- Benefits eligible appointed employees and
- Who have satisfactorily completed six months of continuous service; and
- Who have exhausted all of their accrued sick leave.
Procedure
- Employee submits a request letter to department head including the reason and the leave schedule.
- The department head forwards recommendation to the appropriate dean or vice president.
- The dean or vice president forwards recommendation to the UA President's office for written approval. The response from the President's office is returned to the department.
- If approved, the department forwards paperwork to Human Resources Leave Management Team for benefit tracking purposes.
What Happens to Your Benefits?
- You may continue your health benefits until the end of this leave by paying both the employer and employee cost of any such benefits.
- Following the starting leave date, you will receive in the mail a packet from the Leave Management team of the Human Resources including:
- The form to waive, change or continue the existing benefits
- The bill with the itemized employee benefit cost as well as payment schedule
- Other helpful information
- Continuation of coverage after six months is available through the Consolidated Omnibus Budget Reconciliation Act (COBRA).
