Program Comparison - Long Term Disability

Waiting period before benefits begin Benefits will commence on the day following a Waiting Period during which you have been totally disabled for 6 consecutive months. Benefits will commence the later of any accumulated sick leave or 180 days from date disability began (elimination period)
Maximum length of benefit Benefits are payable for the remainder of your period of disability. (see plan booklet for exclusions and limitations) If you are under age 62 the maximum benefit period ends on your 65th birthday (see plan booklet for a disability that begins after age 62.)
Pre-existing condition restrictions If hired on or after July 1, 1988 a pre-existing condition is any injury, sickness, or pregnancy for which you received medical treatment within 3 months before the date of your LTD coverage effective date. This exclusion does not apply to a disability commencing after you have been a full-time permanent employee for a period of 12 consecutive months. Will not pay benefits for any period of disability caused or contributed to by, or resulting from, a Pre-existing Condition. This limitation also applies to any added benefits or increases in benefits. This limitation will not apply to a period of Disability that begins after you are covered for at least 12 months after your most recent effective date of insurance.
What is not covered If a disability is a result of any of the following: an intentionally self-inflicted injury; war, whether declared or not; an injury incurred while engaged in a felonious criminal act or enterprise; pre-existing condition restrictions, it is excluded from coverage under this plan. If disability results directly or indirectly, from: suicide, attempted suicide, or self-inflicted injury while sane or insane, war or any act of war, whether or not declared, terrorism or active participation in a riot, injury or sickness while you were serving on full-time active duty in any armed forces, commission of a felony, the revocation, restriction or non-renewal of your license, permit or certification necessary to perform the duties of your occupation unless due solely to injury or sickness otherwise covered by the Policy, it is excluded from coverage under this plan. No benefits are payable for any period of Disability during which you are incarcerated in a penal or corrections institution.

Work Earnings Integration If you are working a limited duty due to an illness or injury you may be eligible to receive LTD income benefits. You may work for wage or profit while Disabled. (See plan booklet for Work Incentive calculation)
Medical, Dental & Vision Benefits Once approved for LTD you may continue benefits (with a subsidy, if eligible) through ADOA LTD/Retiree Saguaro Program Once approved for LTD you may continue benefits through ADOA LTD/Retiree Plan. There is not a subsidy afforded.
Life Insurance Benefits Apply for Waiver of Premiums for Standard, Aetna and Reliastar Life Insurances OR Convert Term Life into a Whole Life Insurance Plan.
