You selected Arizona State Retirement System (ASRS) as your retirement plan. To complete your election of the ASRS plan:

  (contains Retirement Plan Election form, ASRS Enrollment form, and ASRS Beneficiary form)


2. Complete all required information on the forms, sign, date and return forms to:

To complete your retirement plan election, forms must be recieved in the Human Resources office within your 30-day election period.
