- How can I find a list of qualifying and non-qualifying expenses?
A list of qualifying and non-qualifying expenses for both the Health Care and Dependent Care FSA can be found on ASI’s website. - How can I view what benefits I currently have?
To view your current elections, log in to UAccess Employee with your UA NetID and password.
Once you have successfully logged in to UAccess Employee, select Employee/Manager Self Service > Self Service > Benefits > Benefits Summary.
- Can I enroll in the FSA if I am currently enrolled in the Health Savings Account Option?
You may only enroll in the Limited Health Care Flexible Spending Account and the Dependent Care Flexible Spending Account. - What is the Limited Health Care FSA?
The Limited Health Care FSA is for employees enrolled in the Health Savings Account Option (HSAO). It can be used to pay for qualified dental, vision and out-of-network preventive care expenses only. Medical expenses other than preventive care are not eligible for reimbursement with the Limited Health Care FSA. The enrollment system will only permit employees enrolled in the HSAO to elect the Limited Health Care FSA. - When will the changes that I make become effective?
Flexible Spending Account Open Enrollment changes/updates will become effective January 1, 2010. - How much will I save in taxes each year by participating?
Use ASI’s tax calculator to estimate your tax savings from enrolling in an FSA. - Is there a deadline for submitting claims?
Claims incurred during 2010 must be submitted by April 30, 2011. - Am I eligible for the debit card? The debit card is an option available to anyone who enrolls in the Health Care FSA. It is not available with the Limited Health Care FSA or Dependent Care FSA.
- How do I elect the debit card?
ASI will send debit card enrollment forms to your home address. If you wish to elect the debit card, completed forms should be returned to ASI. It will take approximately two weeks to receive the debit card after ASI has received your completed application. - When can I use the debit card?
You can use the debit card to pay for qualified expenses at certain grocery stores and pharmacies or at doctors' offices. The expenses will be debited from your Health Care FSA election. A complete listing of stores and pharmacies that can accept the debit card is at www.asiflex.com/debitcards. - Do I have to use the debit card if I elect it?
No, use of the card is optional. If you opt not to use it, you can submit claim forms and documentation to ASI for reimbursement. - Is there a cost associated with the debit card?
Yes, the debit card will cost $1 per month. The annual amount will be deducted from your Health Care FSA annual election in full and is non-refundable. You may elect the card at any time and will only be charged for the months remaining in the year at the time you elect the card. It is important to consider this cost when making an annual Health Care FSA pledge. - If I use the debit card, will I still need to submit paperwork to ASI?
You may still need to submit follow-up documentation to ASI. Most co-pays and over-the-counter purchases can be electronically substantiated and will not require follow-up documentation. However, some out-of-pocket expenses will require follow-up documentation. ASI will send a request to your home addess if follow-up documentation is required. You should always keep your documentation even if ASI does not request it. - Who should I contact with questions about the FSA Plan or the debit card?

To enroll, go to UAccess Employee and log in with your UANet ID and password. Select Employee/Manager Self Service > Self Service > Benefits > Benefits Enrollment. - How do I ensure that my enrollment will be accepted and processed?
You must click "Submit" to finalize your elections at the end of the enrollment process. Once your elections are finalized, they will be processed. - Can I re-enter the system and change information I have already saved?
Yes, you may re-enter the system to make changes or additions as many times as you need to between November 2 through November 30, 2009. - Is the enrollment system available 24 hours a day?
The UAccess Employee System may be brought down for maintenance on Tuesdays and Thursdays from 10:00 p.m. - 12:00 a.m. and Fridays from 6:00 p.m. - 6:00 a.m. During these times, if maintenance is not needed, the system will remain operational and accessible. - What should I do if I have questions or problems with the system?
