Limited Health Care Flexible Spending Account (FSA)
The Limited Health Care FSA is for employees who are enrolled in the Health Savings Account Option (HSAO) with Aetna. IRS regulations prevent contributions to a full Health Care FSA and a Health Savings Account (HSA) in the same calendar year. To comply with these regulations, the Limited Health Care FSA was added this year for HSAO participants who wish to put tax free money aside for reimbursement of qualified dental, vision and out-of-network preventive care expenses . Medical expenses other than preventive care are not eligible for reimbursement with the Limited Health Care FSA.
Optional Debit Card
Starting in 2010, if you enroll in the Health Care FSA, you will have the option of signing up for the FSA debit card. The debit card can be used to pay for eligible expenses incurred at doctors’ offices, and certain grocery stores and pharmacies. For a complete listing of grocery stores and pharmacies able to accept the debit card, please visit ASI’s website. The debit card is not available with the Limited Health Care FSA or the Dependent Care FSA.
The debit card can reduce paperwork for most co-pays and over-the-counter purchases. However, depending on the expense, follow-up documentation could be required by ASI. ASI may send requests for follow-up documentation after the purchase has been made. It is important to always keep follow-up documentation even if ASI does not request it.
The debit card will cost $1 per month. This annual fee will be deducted in full from your Health Care FSA annual election and is non-refundable. You may elect the card at any time and will only be charged for the months remaining in the year at the time you elect the card. It is important to consider this cost when making an annual Health Care FSA pledge.
ASI will mail a debit card application along with your FSA enrollment confirmation to your home address in late December/early January. If you wish to sign up for the debit card, you will need to complete the application and return it to ASI. It will take approximately two weeks to receive your debit card once ASI has received your application.

Claims Submission Deadline
For the 2010 FSA plan year, the deadline for claims submission to ASI will be changing from March 31 to April 30. Any claims incurred during 2010 must be submitted to ASI for reimbursement no later than April 30, 2011. However, claims incurred during the 2009 plan year will still have a submission deadline of March 31, 2010.
EmplID
Effective immediately, all claims (including claims incurred in 2009) should be submitted to ASI using your EmplID. If you do not know your EmplID, please visit https://newid.arizona.edu/.
