NOTE: This form can be printed on regular paper.
Purpose
- The Supplemental Compensation form for Classified Staff is used to authorize compensation beyond the employee's regular assignment. Please refer to Classified Staff Personnel Policy Manual, 316.0 Supplemental Compensation for the definition of Classified Staff work assignments that qualify for Supplemental Compensation.

Main Campus departments: If you have any questions, please contact your Employment Advisor.
Instructions
- Indicate if the request is Original, Revision (revising an existing request), Continuation (extending an existing request).
- Primary Position Information: Complete the Employee Name, etc. with the employee's current employment information, as defined in PSOS.
- Supplemental Compensation Information: Complete with the proposed supplemental compensation information. Payperiod dates are available at:

Approvals
Obtain the following signatures:- The Department Head of the employee's home department.
- The Department Head of the department authorizing the supplemental compensation.
- The Home Department Dean/Director.
- Fund Accountant(s) for the accounts to be charged.
Routing
- Retain a copy for your records.
- AHSC Departments: Send the signed original form to the AHSC Human Resources office, AHSC room 2233.
- Main Campus Departments: Send the signed original form to Human Resources, USB Room 220.
