Instructions
Indicate if the request is Original, Revision (revising an existing request), Continuation (extending an existing request).
Primary Position Information: Complete the Employee Name, etc. with the students's GA Appointment or Summer Instructor position information. This is available from PSOS, screen 03.
NOTE: If the student isn't in a GA Appointment or Summer Instructor position, they cannot be paid on a Graduate Assistant/Associate Supplemental Compensation form.
Supplemental Compensation Information: Complete with the proposed supplemental compensation information.

The Department Head of the department requesting supplemental comp.
Routing
Send the signed original to Systems Control for processing. Retain a copy for your records.
